Georgia State University intends to provide a safe and healthy environment for all students. That is why, in compliance with the State Board of Regents immunization requirements, all students must show proof of being immunized.
These immunizations need to be documented on the Immunization form (Certificate of Immunization) found on this page.
Students can request records of their immunizations from schools that they have previously attended, county health departments, military records or their physician.
All vaccinations or history of disease must be validated by a licensed health care provider with a signature and medical office stamp. Parent or guardian notes for history of disease are not acceptable forms as the requirement must be validated from a health care professional and a medical office stamp.
Students residing in campus housing are required to receive the meningococcal conjugate (MenACWY) vaccine or sign a waiver. Housing residents will complete a form prior to or upon move-in indicating their vaccination status.
All students attending Georgia State University are required to submit a written or electronic healthcare provider record to the Student Health Clinic of their required immunizations.
Students who are less than 19 years old at the time of registration are also required to have received the hepatitis B vaccine series.
Students residing in campus housing are required to receive the meningococcal conjugate (MenACWY) vaccine.

Immunization Record Processing
Students should allow four business days from the time their immunization records are received for processing, posting and releasing immunization holds. Incomplete or unacceptable records may take up to 14 business days due to correspondence time requesting corrections. Common reasons for incomplete immunizations records are no valid Panther ID, no physician signature, no physician stamp or unreadable dates.
Unable to Locate Immunization Records
Students who are unable to locate their immunization records but are certain that they have had the immunization should have blood tests (titers) done instead of repeating the shots. If the titer shows that the student is immune, then he or she has proven immunity. If the titer shows a student is not immune, he or she will need to receive the immunization. Proof of tetanus and diphtheria is by immunization record only; there is no titer for this immunization.
Students may be exempt from receiving immunizations for a variety of reasons. We require documentation of each of these exemptions, which may be provided in the form of a waiver.
Medical Exemptions: Accepted with a written note from student's health care provider, on their letterhead with their signature.
- Permanent: Permanent medical contraindication (ex: allergy).
- Temporary: Exempt for a period of time (ex: pregnancy).
Religious Exemptions: Immunizations may conflict with someone's religious beliefs. A student may be subject to exclusion from school in the event of an outbreak of a disease for which immunization is required if they opt to receive a religious waiver. Students must submit the completed and notarized Affidavit of Religious Objection to Immunization form, in addition to the Certificate of Immunization. Students must sign that they acknowledge the assumption of risk in the event of an outbreak on campus.
Distance Learning Exemption: Students enrolled only in courses offered by distance learning (online). If the student enrolls in a class on campus, the exemption is void.
Military Exemption: Students seeking military exemptions must show their DD214 form or an equivalent military discharge form.

All students attending Georgia State University are required to submit a written or electronic healthcare provider record to the GSU Student Health Clinic confirming immunization to measles, mumps and rubella (MMR); varicella; and tetanus, diphtheria, and pertussis (Tdap).

Vaccination or immunization records provide a history of all the vaccines someone has received. This record may be required for certain jobs, travel abroad or school registration.
Students who would like to obtain a copy of their immunization history at Perimeter College at Georgia State University and entered Perimeter College at Georgia State University after spring 2015 can obtain their immunization records through the Patient Portal. Students should follow the link and utilize their student ID number and the appropriate password to log in to the portal. Once logged into the portal, select Immunizations and then select History. On the history page, select Print.
Upload completed forms and, if applicable, lab results on the Upload Your Forms page of the Patient Portal. Please upload forms and lab results as a PDF file. There is a 4MB file size limit.
No. Transfer students will need to contact their former institution to obtain copies of their immunization records. An Authorization to Disclose form can be obtained and submitted to retrieve immunization records. Students who do not meet Georgia State immunization requirements will have to take necessary vaccines or show proof of immunity to comply with the vaccination policy.

The purpose of this policy is to ensure that students are protected against communicable diseases that are preventable and to reduce the likelihood of an epidemic or threatened epidemic on a Georgia public college or university campus.
Student Health Clinics will not be able to verify immunization data. Students' records will be incomplete, and a hold will be placed on their student account, which will block them from registering for classes.
Yes. Students who are not in compliance with the Georgia State University immunization policy will have a hold placed on their student account until they have met the requirements.

- Online Cohort Immunization Holds: If you are taking online classes only, please submit the Immunization form by checking the Distance Learning box at the bottom of the immunization form page 1. Those who are are switching from online-only to in-person classes must submit their updated immunization records to be able to register for in-person classes.
